Graduation Thank You Card in german

I'm sending a thank-you note to my relatives in Germany for a graduation present. I would like to say something like:

"Thank you very much for the card and gift for my graduation. It means a lot to me that you've kept me in your thoughts."

Possible german translation:
Vielen Dank für die Karte und das Geschenk zu meinem Schulabschluß. Es bedeutet mir sehr viel, daß Ihr an mich gedacht habt.
